Organise records when the hospital requests them
- A short diagnosis and treatment timeline, with dates.
- Pathology reports and relevant laboratory or molecular reports.
- Recent imaging reports and instructions for accessing the images.
- A current medication list and your oncologist's referral summary, if available.
- Your main questions and any practical constraints on travel.
This is an organisational starting point, not a mandatory clinical checklist. The hospital decides which records are needed.
Agree how records will be shared
Before sending records, confirm the intended recipient, the transfer method and whether translation is needed. Obtain the patient's permission if acting on their behalf. Ask which identifiers are required; do not send passports or full records in an unsolicited first message.
Before making arrangements
Request clarity on the proposed hospital, the purpose of the visit, expected next steps, fees and cancellation terms. Ask what can be reviewed remotely, what requires attendance and who will update you if the plan changes.
